JONESBORO – Extended Family Home Health Has been named 2019 Home Health CAHPS Honors recipient by Healthcare First, a leading provider of billing and coding services, Care Consumer Assessment of Healthcare Providers and Systems surveys and advanced analytics. Home Health Honors is a prestigious program that recognizes home health agencies, providing the highest level of quality as measured from the patient’s point of view.

Extended Family Home Health was established in 1982. It has been serving “Homebound” patients in Craighead and Poinsett counties for over 37 years.

The company provides skilled nursing, physical therapy and certified nursing aides to help with personal/hygiene needs.

If you or your family members need help to stay at home, we’re here for you. Seniors, veterans, children and adults with disabilities and employees temporarily off work due to injury all count on us to provide a full range of care.

Services Include:

Our nurse comes to your home to provide many of the medical services you’d normally get at a doctor’s office or clinic—injections, wound dressing, monitoring vital signs, catheterization, medication review, dietary teaching and much more.

Our health care aid helps with hygiene, feeding, changing linens and other necessary household chores. As a member of your personal care team, the aid looks out for you by recording vital signs and observations for your care team. She’s also your biggest cheerleader, encouraging you to meet your goals and celebrating when you do.

We bring the rehab to your home as well as develop a care plan that meets your goals. Our licensed therapist will make your safety top priority and collaborate with you, the patient, to help you get back to what you enjoy doing most.

The Occupational Therapist evaluates your home and helps with adaptations to make it work better for you.

Our licensed professional will evaluate your speech, hearing and cognition abilities. And from that information, we will develop a plan of care for you to help maintain and improve your communication and cognition skills. At Extended Family Home Health we are proud to offer a speech therapists that is Vital Stim certified. This is a non-invasive therapy that can help improve certain swallowing issues.

When we begin care, we carefully assess not just your needs but also your likes and dislikes. And we respect your choices. There’s little turnover in our caregivers, so we’re around long enough to get to know you, even your pets and favorite TV shows.

Your physician is in charge.

Your physician must sign off on your individual and detailed treatment plan. If you need help taking care of this part, we’re there for you all the way. Throughout your treatment, we’ll report to your physician so he or she can stay on top of your progress.
